Email address of the person making this request on behalf of the new user.
Title of the person making this request on behalf of the new user.
The Company name
Secondary location for the new user if they will be based somewhere other than the main location.
New User Name
Please specify how you would like the new user's email address to be formatted and please include the domain.
Display name" refers to the name that will be visible to other users in the system, typically consisting of the user's first and last name combined, which you can customize when setting up the account; it's essentially the preferred name that will appear in emails, team meetings, and other Microsoft 365 applications.
Then name of the Supervisor or Manager of New User
This designates the specific physical location to which the user will be assigned.
This information is required for security setup in Microsoft 365.
A public email address (e.g., Gmail, AOL, Yahoo) is required for MFA security and to send the end user their temporary credentials.
This is the primary phone number for the assigned physical site.
New User Assigned Product Licenses
Each Microsoft 365 user license includes a range of services, from web-only access to full desktop and web versions of the 365 apps. By selecting the appropriate boxes indicating your requirements, you'll assist us in determining the most suitable license for your needs.
SharePoint consists of sites with data repositories (libraries). If you know the sites the user needs access to, enter it here. Default permissions will be set to 'member' for all Public areas; provide additional details for higher-level permissions.
Microsoft Teams is organized into channels and rooms. Channels can be general, while specific rooms might be designated for teams like accounting or sales. If you know the channels and rooms the new user will require access to, please enter them here.
